2. Provide your personal specimen for scanning. Acceptable specimens: hair or nail clippings. (Please cut 12 1-inch strands of your hair or save all your nail clippings).
3. Enclose your specimen in plain paper. Make certain to fold all four sides & secure.
4. Label with your full name, date of birth, email, mailing address, phone.
5. Enclose your secured & labeled specimen, together with your completed Order Confirmation with electronic payment, in a mailing envelope. Send OVERNIGHT to the following mailing address:
